Complaints form self-assessment

Before you can submit a privacy complaint to us, you must first complete this self-assessment. It steps you through whether your complaint is one we are able to investigate and/or if there is any action you need to take first. 

For us to be able to assess your complaint, you will need to attach the following documents:

  • A copy of your request or complaint to the agency
  • A copy of their response
  • A copy of your follow up emails to the agency (for example following up when you have not got a response from the agency)
  • Any evidence of the breach you are alleging
  • If relevant, evidence of your harm. 

Giving us the key facts in your own words (don't use AI) with supporting evidence is the best way to help us to help you. If you do not provide this information, we will not be able to assess your complaint. 

Other ways to raise a concern
If you'd like to raise a concern with us, but you don't want us to contact the agency, or to identify you to the agency you can email us (anonymously if you wish) at enquiries@privacy.org.nz 

If you use our complaint form, we will assume you wish us to take action on your complaint, and this will usually involve contacting the agency and asking them to resolve your concerns.
